Data collection

Data is collected from you, your veterinary surgeon and other paraprofessionals involved with the treatment of your animal.

 

Veterinary consent/referral forms

Physiotherapy is only undertaken following receipt of a completed veterinary referral/consent form in accordance with The Veterinary Surgery (Exemptions) Order 2015 (SI2015/772) in the Veterinary Surgeons Act (1966).  These forms are stored on a password protected OneDrive cloud account; access to this account is granted solely to Charlie Quinn of Animate Veterinary Physiotherapy.

Physiotherapy treatment reports

These are medical documents produced to document the treatment of your animal and monitor progress. This information is shared with your veterinary surgeon and other paraprofessionals involved in the treatment of your animal.  In accordance with insurance requirements, the records shall be kept for at least 7 years following the last occasion on which treatment was given.  

 

Contact details

Addresses, telephone numbers and email addresses are stored in a password protected Samsung account with your first name, surname and your animals’ name.  All electronic devices are password protected and linked to a OneDrive cloud account as security. 

 

How else may we use your data

You may receive emails/texts/WhatsApp messages from time to time with special offers and services Animate Veterinary Physiotherapy think you may like. You have the right at any time to stop Animate Veterinary Physiotherapy contacting you for marketing purposes.

 

Photographs and videos

Photographs and videos of your animal(s) may be taken and stored to enable assessment of progress during ongoing treatment. Any such photos or videos are stored on a password protected OneDrive cloud account and are only shared on social media and/or our website with your permission.

 

Your data protection rights

Every client is entitled to the following;

• To request copies of your personal data from Animate Veterinary Physiotherapy.  We may charge you a small fee for this service

• You have the right to request that Animate Veterinary Physiotherapy correct any information you believe is inaccurate.  You also have the right to request we complete any information you believe to be incomplete.

• You have the right to request that Animate Veterinary Physiotherapy erase your data under certain conditions and with due consideration of our legal requirements.

• You have the right to request that Animate Veterinary Physiotherapy restrict the processing of your personal data, under certain conditions.

• You have the right to object to Animate Veterinary Physiotherapy processing your personal data, under certain conditions.

• You have the right to request that Animate Veterinary Physiotherapy transfer the data that we have collected to another organisation, or directly to you, under certain conditions.
